We are committed to protecting your personal information and handling it responsibly in accordance with the Australian Privacy Act 1988 and the Australian Privacy Principles.
We collect information you provide directly to us, including:
• Your name, email address and location when you submit an application or enquiry through our website
• Health and medical information provided through our client intake and consultation forms - collected for the purpose of providing safe and appropriate treatments
• Booking and payment information processed through Acuity Scheduling
• Photographs taken during or after treatments, where you have provided explicit consent
We use your information to:
• Respond to your enquiries and manage your bookings
• Provide safe and individualised treatments
• Send appointment reminders and follow-up communications
• Maintain accurate client records as required by our professional obligations
We do not use your information for marketing purposes without your consent and we do not sell your personal information to any third party.
Third parties
We use Acuity Scheduling (a Squarespace product) to manage bookings and payments. By booking through our scheduling system you are also subject to Acuity's privacy policy, available at acuityscheduling.com.
We do not share your personal information with any other third parties except where required by law.
Photography and content
We may request your permission to photograph or film during or after a treatment for use on our website or social media. This is entirely voluntary. You will be asked to provide explicit written consent before any photography is taken or used, and you may withdraw this consent at any time.
Data security and retention
Your personal information is stored securely and accessible only to Laura McCarthy. Health and consultation records are retained for a minimum of seven years in accordance with Australian health practitioner guidelines. You may request access to or deletion of your personal information at any time by contacting us at hello@forme.au.

Bookings
All appointments are booked through our online scheduling system at forme.au. Full payment is required at the time of booking to secure your appointment. Bookings are not confirmed until payment has been received.
By completing a booking you confirm that you have read and agree to these Terms & Conditions.
Cancellation policy
We understand that circumstances change. Our cancellation policy is as follows:
• Cancellations made more than 48 hours before your appointment will receive a full refund.
• Cancellations made within 48 hours of your appointment are non-refundable.
• In the event that Laura cancels your appointment, you will receive a full refund promptly and be offered priority rebooking.
To cancel or reschedule please contact us as soon as possible at hello@forme.au or through your booking confirmation.
Health and safety
You are required to complete a client consultation and health history form prior to your first treatment. It is your responsibility to disclose any relevant health conditions, medications or contraindications. Laura reserves the right to modify or decline a treatment where it is not considered appropriate or safe.
If you are pregnant, have recently had surgery, or have any active skin conditions or infections please contact us before booking.
Session locations
Sessions are held at private locations which are confirmed after booking. Location details will be shared with you via email once your booking is confirmed. Please do not share location details with others.
Punctuality
Please arrive on time for your appointment. Late arrivals will result in a shortened treatment time and the full fee will still apply. If you are running more than 15 minutes late please contact us - we will do our best to accommodate you where possible.
Results
Results from facial treatments vary between individuals. Laura does not guarantee specific outcomes. All treatments are provided in good faith based on the information available at the time of your session.
